All students that are completing work experience while in Coláiste an Átha must fill out the following paperwork. This ensures that they will have correct insurance while on their placement.
Form 1 must be filled out by your parent/guardian. Student Learner Work Placement Agreement
Form 2 is for the employer. Work Placement Provider Agreement Form
Form 2 can be emailed to the employer if it is easier.
Both forms must be completed and returned to the school and an insurance letter will be issued for the work placement. Students are not permitted to attend work experience without an insurance letter.
